What You’ll Do
- Serve as the primary RN Case Manager for hospice patients with life-limiting illness
- Provide skilled nursing care to patients in private homes and long term care facilities.
- Manage pain control, symptom management, wound care, and medication oversight
- Support patients and families through end-of-life decision-making with clarity and compassion
- Coordinate care with physicians, social workers, chaplains, CNAs, and the interdisciplinary team
- Educate caregivers on disease progression, comfort measures, and hospice services
- Maintain accurate, timely, and compliant documentation in the EMR
- Help shape workflows and care processes as the agency continues to grow
Who Thrives in This Role
- Active Minnesota RN license
- 2+ years of experience required in hospice, oncology, palliative care, end-of-life care, or complex geriatric nursing
- Strong background in home care, assisted living, SNF, or hospital-based care
- Confident clinical decision-maker with excellent critical thinking skills
- Emotionally grounded, compassionate, and comfortable supporting patients at end of life
- Organized, self-directed, and comfortable working independently in the field
- Committed to ethical, patient-centered, and regulation-compliant practice
